DNA Hakim agreement cover

1. Purpose

This agreement defines the professional framework governing in-clinic consultations and online health coaching services provided by Dr Mohammad Rashid Khan, DOH & MOH Licensed Practitioner, Abu Dhabi.

The purpose of this document is to establish transparency, ethical practice, informed participation, and patient safety in compliance with UAE healthcare regulations.

2. Scope of Services

  • • Unani constitutional (Mizaj) assessment
  • • Review of medical history and diagnostic reports
  • • Personalized lifestyle and dietary guidance
  • • Preventive and wellness-oriented planning
  • • Nutrigenomic interpretation (where applicable)

Services provided are advisory, educational, and preventive in nature and are not a substitute for emergency or hospital-based care.

3. Patient Declaration

  • • All health information shared is accurate and complete
  • • Existing diagnoses, medications, and therapies are disclosed
  • • Emergency medical care will be sought when required
  • • Recommendations are guidance-based, not guarantees

Clinical interpretations and recommendations are based entirely on the information provided by the patient.

4. Confidentiality

All personal and medical information is treated as confidential and managed in accordance with applicable healthcare data protection regulations.

Records are securely stored and will not be disclosed without patient consent unless legally required.

5. Limitations

No guarantees, assurances, or promises of cure are made. Health outcomes vary based on individual physiology, compliance, lifestyle, and underlying conditions.

Genetic and nutrigenomic interpretations do not constitute medical diagnosis or disease prediction.

6. Appointments & Fees

Appointments are confirmed only upon successful booking and payment, where applicable.

Missed or unattended sessions without prior notice may not qualify for refund or rescheduling.

7. Telehealth

Online consultations are conducted via secure digital communication platforms.

Telehealth services may have inherent limitations compared to physical clinical examination.

8. Liability

The patient accepts responsibility for the implementation of lifestyle, dietary, and wellness recommendations.

The practitioner is not liable for outcomes arising from non-compliance, inaccurate disclosures, or omitted history.

9. Governing Law

This agreement is governed by and interpreted in accordance with the healthcare laws and regulations of the United Arab Emirates.

10. Informed Consent

By proceeding with booking, the patient confirms that they have read, understood, and voluntarily accepted all terms outlined in this agreement.
